SIMS Hospital Performs Total Jaw Joint Replacement Surgeries Using 3D Printed Joints!


Ushering a new era in jaw joint replacement in Tamil Nadu, SIMS Hospital, Vadapalani a reputed multi-speciality hospital in Chennai, has successfully performed rare surgeries of total jaw joint replacement using 3D-printed as well as readymade jaw joints for three patients, recently. Though surgeries for the replacement of hip and knee joints have become common, the ones for the replacement of jaw joints and the use of 3D printed custom-made joints are new.

Explaining the uniqueness of the procedure, Dr. K. Sridhar, Director, Institute of Craniofacial, Aesthetic & Plastic Surgery, SIMS Hospital, said:


“We have performed total jaw joint replacement for three different patients. In one patient, who had zero mouth opening following a jaw surgery done elsewhere, the damaged jaw bone part was replaced with an artificial part made of titanium metal.

 

The same was done for the second patient who had complete ‘ankylosis’ (immobile joint) of temporomandibular joint (TM joint), which acts like a sliding hinge, connecting jawbone to skull. This patient also had the complaints of sleep apnoea (snoring and sleep disorder due to less oxygenation) and hypoplasia of jaw bone (smaller than normal jaw bone). For this patient, first the jaw bone was lengthened with distraction and later total joint replacement was performed.

 

The third patient had a bone tumour, for which the entire one half of the jaw, the TM joint and the adjoining facial bones were removed about 20 years ago. This patient required a custom made jaw joint for replacement. Since no bone was available to fix the joint, the total jaw joint was replaced with artificial jaws made of titanium and polyethylene using 3D printing technology. Since this patient required teeth on one half we also took leg bone and did microsurgery and put in a dental implant also at the same time.”

 

All the patients now have very good mouth opening. The two patients who were not able to open their mouths at all are on solid food after many years. The third patient with a custom made joint is waiting for the fixing of the teeth on the implant.

 

Talking about the necessity of jaw joint replacement, Dr Sridhar said, “Patients who have problems with opening of mouth (Trismus) due to infection, arthritis or trauma, struggle to chew their food. In extreme cases, the patients can hardly achieve 2–3 mm of mouth opening. We call the condition of immobile joint as ‘ankylosis. Those who have ankylosis will not be able to take real solid food, and will be forced to live their lives only with a soft or semi solid diet.”

 

Usually, the treatment for ankylosis involves creation of a gap by cutting the fused joint and tissues (like muscle, cartilage, or silicone) to prevent fusing again. But, recurrence and failure are very common. In the case of cancer, the adjoining bone may be removed leaving behind a large gap. The reconstruction for it is done by transferring a bone from leg to jaw with micro-surgery. This is possible if a joint is present. If the joint is also removed, then there is a need to create a new joint.

 QuestionCloud.in Announces Free Online Mock Test




In what comes as a boon for the students of Government schools in the State, who aspire to crack NEET exam in their mother tongue of Tamil, the Chennai-based QuestionCloud.in, India’s largest online educational assessment portal, has announced today (20th Aug. 2021) that it is making its mock test series for NEET free for them.

 

Students can register in the portal (www.questioncloud.in) and try NEET mock tests in Tamil any number of times using their mobile phone or computer. The portal submits the results instantly, with which the students can assess their own preparedness for the NEET exam in an objective way, all without having to pay any fee. 

 

A brainchild of Dr. Akilan Ramnathan, a former bureaucrat, questioncloud.in has over 7,500 different tests containing over three lakh questions. The portal is developed by Blue Silicon Infotech Private Limited, an edutech startup promoted by him. As for school exams, the portal has mock tests from Class 6-12 students of CBSE and State Board in Tamil and English mediums.

 

QuestionCloud.in also has tests for different jobs in the Public Sector Companies, State and Central Government Departments. The mock tests currently cover TNPSC, other State PSCs, UPSC, SSC, IBPS, RRB, Banking and Defence Services Exams.

 Hydrogen Fuel Cell-based Train on Indian Railway Network!

Indian Railways is exploring the feasibility of retrofitting the trains (that are presently run on diesel) with hydrogen fuel cell-based technology.  Indian Railways Organization of Alternate Fuel (IROAF), Green Fuel vertical of IR, has invited the bids for this demonstrative project in 89km Sonipat-Jind section of Northern Railway. This initiative is part of the National Hydrogen Energy Mission, to launch and promote the concept of Hydrogen Mobility in India.  Rs.8 crore has been allocated for the project in 2021-22.

Initially, two DEMU rakes will be converted, and later two-hybrid narrow gauge locos will be converted based on hydrogen fuel cell power movement. After conversion, the train will run on hydrogen fuel, the greenest mode of transport as hydrogen can be generated by electrolysing the water from solar energy.  

The retrofitting of diesel-powered Diesel Electric Multiple Unit (DEMU) and converting it into hydrogen fuel-powered train set will not only save the cost to the tune of Rs 2.3 crore annually by converting from diesel to hydrogen, but also save the carbon footprint (NO2) of 11.12-kilo tons per annum and particulate matter of 0.72-kilotonnes per annum.

The bidding date for hydrogen fuel cell-based DEMU rake will start on September 21, 2021, and the closing date will be October 5, 2021. Hydrogen fuel cell-based technology is part of “Advanced Chemistry Cell (ACC) Batteries” and “National Hydrogen Mission”, the two flagship programs of the Government of India  to meet out the targets of reducing Green House Gas (GHG) emission under Paris Climate Agreement 2015 and “Mission Net Zero Carbon Emission Railway” by 2030.
